Autologous fat grafting of lower eyelid

2. Autologous fat filling lower eyelid surgery process

First, mark the extent of the depression and the aspiration range of the fat donor area. The fat donor area is generally chosen near the navel in the lower abdomen. According to the size of the depression, determine the minimum suction range and the amount of suction. The fat donor area is anesthetized locally with local infiltration, and suction is performed using a 5ml syringe and a No. 16 syringe needle. When suctioning, first draw 1ml of normal saline into the syringe. The saline can act as a buffer during suction, thus reducing bleeding. Make a small incision (1mm to 2mm long) at the edge of the suction area. After the needle pierces the skin through the incision, draw the syringe into negative pressure and maintain it. Repeatedly use several syringes to aspirate until the estimated amount of fat particles is sufficient. Choose a more hidden place on the edge of the depression (1mm to 2mm) and make an incision. The needle penetrates into the subcutaneous tissue through the incision and the fat particles are evenly injected into the skin. The injection volume should exceed about 50% of the required filling volume. Finally, the incision was sutured with 5/0 silk sutures and removed after 6 days. Fat absorption will be obvious within 1 to 2 weeks and basically completed after 1 month, which can relieve the problem of sunken lower eyelids.

3. Notes

It should be noted that part of the autologous fat will be absorbed. If some patients do not achieve satisfactory results after surgery, they can be injected again after 3-6 months. Fat suction should be performed in the subcutaneous layer, not too deep, and long needles should not be accidentally inserted into the abdominal cavity. When performing liposuction and injection, special attention should be paid to aseptic operation to avoid infection leading to surgical failure.
