Nevus, also known as nevus cell nevus and melanocytic nevus, is a benign disease characterized by color changes of the skin and mucous membranes due to the proliferation of nevus cells and the production of pigments. Pigmented nevi remain stable for a long time and do not recur after surgical removal; but we should be alert to the fact that a very small number of pigmented nevi may become malignant. Pigmented nevi and pigmented spots are collectively called pigmented nevi, which are localized abnormal pigmentation lesions. Pigmented spots, also known as non-cellular pigmented nevi, are pigmentation of the skin without tumor cells, such as freckles, age spots, etc.; unlike pigmented spots, pigmented nevi are the proliferation of nevus cells, so they are also called cellular nevi. Clinically, it is difficult to distinguish between pigmented spots and nevi. The nevus cells in pigmented nevi develop from neural crest precursor cells and are capable of producing pigment. Pigmented nevus is a benign disease that maintains a stable state for a long time and it is very rare for it to become malignant. Pigmented nevi often occur on the skin of the face and neck, and are occasionally seen on the oral mucosa. Disease classification (1) Intradermal nevus: the most common. It often occurs on the facial skin and hairline, and varies in size. The largest can reach several centimeters or even affect half of the side of the neck. The skin is raised, hairy, light brown or light black, with clear borders. Made up of small mole cells. Nevus cells nest in the subepithelial connective tissue (ie, in the dermis). The epithelial layer overlying the nevus cell nests was normal. Generally, cancer does not occur. (2) Junctional nevus: It can evolve from an intradermal nevus or occur alone. It can occur at any age, but is more common in infants and young children. The lesions are flat, brown-black or bluish-black, with clear or unclear borders. Generally, they are small in size, mostly within a few millimeters. The surface is smooth and hairless. It can remain unchanged for a long time, and a few may disappear on their own, but it is more likely to become malignant. It is composed of large nevus cells, half of which are in the bottom layer of the epidermis, and the other half are in the superficial connective tissue under the epithelium, that is, the superficial dermis. There is a possibility of cancer. (3) Compound nevus: It is more common in children. Most of the lesions protrude slightly from the skin surface. A few have papilloma-like changes. They are generally hairless and may increase in size and darken in color with age. The junctional nevus part of a compound nevus may undergo malignant transformation. The pathological characteristic is that in the process of nevus cells entering the dermis, there are often intradermal nevi and residual junctional nevi at the same time, which is a mixed form of intradermal nevi and junctional nevi. Disease treatment The vast majority of moles do not require treatment. Moles on the face that affect appearance can be surgically removed. When the area is large, it can be removed in stages, or it can be removed in one go and then free skin grafting or adjacent skin flap transfer can be performed. If a mole is suspected to be malignant, it should be completely removed and biopsied at once through surgical operation; the incision should be made on the normal skin outside the mole boundary. For example, after a smaller mole is removed, the skin edges can be peeled off secretly and then sutured directly. In short, if the mole is formed by acne, then it must be treated as soon as possible. After all, the mole is a kind of pigmented mole. If it is not treated in time, it may become malignant.
