Treatment of pyoderma gangrenosum

Western medicine treatment

Once the diagnosis is confirmed, high-dose oral corticosteroids: 1-3 mg/kg, or methylprednisolone pulse therapy: 1g/d for 5 days, added to 150ml of 5% glucose solution, intravenous drip, the drip is completed in 1 hour. Salazopyrin, sulfapyridine, or cytotoxic drugs may be used as adjuncts in some cases. Salazopyrin has antibiotic, immunosuppressive and anti-prostaglandin effects and is used at a dosage of 4 to 6 g/d. Dexamethasone can also be injected intralesionally, but the efficacy varies. The drain may be covered with a synthetic resin dressing. A hyperbaric oxygen chamber can reduce pain and speed up healing. There are successful examples of treatment with potassium iodide. Hexenolone has anti-TNF effects and can also be used. Patients with related diseases should be treated for the related diseases, including using milphalan to treat myeloma. Cyclosporine 10 mg/(kg/d) can also be used. Chlorphenazine 200-400 mg/d can be used to control the condition. If there is no secondary infection, local treatment should be mild and symptomatic. In some cases with intestinal disease, colectomy is often ineffective. Minocycline 100 mg, twice a day, is particularly effective for subacute cases. Intravenous human immunoglobulin became popular 30 years ago and has been used again recently.

Prognosis

The prognosis is poor, particularly depending on whether the immune system is involved. However, when ulcerative colitis and Crohn's disease are controlled, the lesions are expected to heal, although they may recur later.

Different diseases have different treatments. Although some diseases seem to have the same symptoms on the surface, they are actually different. After careful examination and description of the symptoms, the conclusions drawn will not be the same. Some symptoms may seem the same, but they must be ruled out one by one before a conclusion can be drawn. You must prescribe the right medicine for the right disease. You cannot be blind or self-righteous. Many illnesses are delayed because of self-righteousness. Being sick is not scary, what is scary is misdiagnosis and missing the best period for treatment.
