Is there a high chance of failure in nose surgery?

Rhinoplasty is a common micro-plastic surgery method for many people. It can make the nose more three-dimensional, improve some congenital nasal underdevelopment, or nose bridge collapse. However, there is a risk of failure in rhinoplasty, which can make the condition more serious. You need to choose a regular plastic surgery hospital. Usually the chance of failure is not very high.

Is there a high chance of failure in nose surgery?

Rhinoplasty failure is caused only by factors such as the doctor's level and improper postoperative care. If the following nine situations occur after your rhinoplasty, it means that your rhinoplasty has failed.

1. Infection:

Most of them are due to incomplete disinfection before surgery and improper operation during surgery. Nowadays, surgical failure due to infection is rare.

2. Changes in skin color:

It is a common problem after surgery, characterized by redness and shiny skin on the nasal dorsum and nose tip. This is mainly caused by the prosthesis irritating the skin of the nasal dorsum for a long time. The degree of skin irritation is related to several factors:

1. It is related to the material of the prosthesis.

The harder the implant, the greater the irritation. Therefore, in order to reduce skin irritation, we prefer autologous tissue in material selection, followed by soft expanded polytetrafluoroethylene. Silicone has a hard texture and is very irritating to the skin, especially the domestically produced silicone that we used in the early days. Since it contains more impurities, it is also more irritating to the skin. The high-quality medical silicone used in recent years is much less irritating to tissues. When using silicone to augment the tip of the nose, be careful not to lift the tip of the nose too much, as this can easily cause excessive tension in the skin at the tip of the nose and affect local blood circulation, causing the skin at the tip of the nose to appear white or red. In severe cases, it can cause the silicone to fall out. Therefore, we all advocate the use of autologous cartilage at the tip of the nose to reduce complications.

2. Related to the level of implantation

The shallower the implantation, the stronger the irritation to the skin.

3. It is related to the thickness and sculpture shape of the prosthesis

The thicker the implant, the more obvious the irritation to the nasal dorsum. Especially for patients with thinner skin, it is necessary to choose an implant that is soft in texture and not too thick. Also, when carving the prosthesis, if the bridge of the nose is carved too steeply, it will cause excessive local pressure, affect local blood circulation, and cause local redness.

3. Prosthesis skewness:

Silicone material has the advantages of being relatively stable and easy to carve and implant, but it is easy to become distorted due to technical reasons.

Causes of crooked implants include:

First, the implant cavity is not symmetrical when it is dissected, and the cavity is naturally prone to deviation when the prosthesis is placed in it.

Second, the cavity is too wide, making it difficult to fix the implant and it can easily move in any direction.

4. Prosthesis displacement:

It mainly occurs when using silicone prostheses. It is caused by the fact that when the prosthesis is implanted under the dorsal nasal fascia, the ventral surface of the prosthesis is not tightly enough combined with the back of the nasal bone. In addition, the texture of silicone is relatively hard and it does not bond well with the surrounding tissues, so it will slip over time.

Many patients do not have a crooked nose when they first have their rhinoplasty done, but may find that their nose is crooked or has slipped 2 years later. Many doctors are puzzled when they encounter this problem, so when carving silicone prostheses, doctors should not only focus on carving the surface of the prosthesis, but also the carving of the ventral surface is also very important.

6. Loosening of prosthesis:

Implantation is too shallow. The correct level should be below the periosteum.

5. Distortion effect:

The main problem is that when carving the prosthesis, the angle of the edge of the prosthesis is too steep or the boundary is too obvious. After a period of time, the boundary can be seen, making the nose clearly look artificial. Especially for those customers with thin skin, they should be more cautious in choosing prostheses, and try to choose soft and thin materials. When processing the edges of the prosthesis, the transition must be natural.

7. Rejection reaction:

The actual chance of rejection is only a few thousandths. The rejection reaction of expanded polytetrafluoroethylene is lower.

8. Obvious incision scars:

This is mainly due to the doctor's choice of incision shape and incision suturing skills. Many doctors choose to make the incision at the edge of the nostril during rhinoplasty. If the incision is not properly positioned, it can easily cause obvious scars or even deformation of the nostrils. To avoid this, when I perform rhinoplasty, I always make the incision inside the nasal dome (inside the nostril, 3 mm away from the edge of the nostril). Although this increases the difficulty of the operation, no scars can be seen from the outside.

When choosing the columella incision, in order to reduce scars, a tortuous incision is used, and the incision skin must be aligned during suturing. When making the columella incision, I use a more complex and tortuous incision. Although it increases the difficulty of the operation, it speeds up the healing process, improves the surgical effect, and makes the postoperative scars difficult to detect with the naked eye. When suturing, try to use imported thread as it causes less irritation to the skin, and try to shorten the time for removing stitches.

9. Unsatisfactory results after surgery:

In the process of dealing with customers, we found that most of them came for repair because they were dissatisfied with the results of the surgery. There are three main reasons for dissatisfaction:

(1) The customer’s expectations are too high, and the results she wants cannot be achieved based on her basic requirements.

(2) The doctor did not communicate enough with the customer and did not fully understand the customer's requirements, so the nose he made was not what the customer expected.

(3) The doctor's technical level and aesthetic sense are not good enough. Many times, customers do not know exactly what kind of nose they want, and they leave themselves completely to the doctor. If the doctor's technical level is not up to standard, the effect will naturally not be ideal. Although some doctors have no technical problems, they do not have good aesthetic standards and are inconsistent with the aesthetic curve of modern people. The noses they make will definitely not be high-quality.
