The sebaceous glands of the hair follicles are blocked by keratin and sebum, while the openings are connected to the outside world, forming open comedones, which appear as black spots of varying sizes on the surface.

3. Papules

It is the most basic symptom of acne. When the opening of the sebaceous gland of the hair follicle is blocked, an oxygen-deficient environment is formed in the sebaceous gland of the hair follicle, and the anaerobic Propionibacterium acnes multiplies in large numbers, decomposing sebum, producing chemical chemokines, and white blood cells gather to form inflammatory papules. Therefore, this type of papule is an inflammatory lesion.

4. Pustules

It is the further development and aggravation of inflammatory papules. A large number of neutrophils gather in the sebaceous glands of the hair follicles, swallowing Propionibacterium acnes to cause an inflammatory reaction, and a large number of pus cells accumulate to form pustules. This condition is prone to form scars after healing, mainly depressed scars.

5. Nodules

On the basis of pustules, a large amount of keratin, sebum and pus cells are stored in the sebaceous glands of the hair follicles, which destroy the structure of the sebaceous glands of the hair follicles and form red nodules that are higher than the surface of the skin. There is obvious infiltration, flushing, and tenderness to the touch at the base.

6. Cyst

On the basis of the nodules, a large number of pus cells gather in the sebaceous gland structure of the hair follicles, which contain pus, bacterial residues, sebum and keratinized substances, as well as inflammatory infiltration. The sebaceous gland structure of the hair follicles is completely destroyed. It feels like a cyst to the touch, and pus and blood may overflow when squeezed.

7. Pyrosis

On the basis of the cyst, all the contents in the sebaceous glands of the hair follicles gradually dry up, the inflammation is reduced, and a greasy, bean-dreg-like substance is formed. When the pressure in the cyst is too high, small holes will form on the surface, from which bean-dreg-like or cheese-like substances can be squeezed out. It is a cystic tumor when touched.

8. Scars

Scars are the most serious lesions of acne. In severe damage above the inflammatory papules, the dermis is destroyed and the connective tissue is repaired after healing to form scars. Scars are the body's repair response to tissue damage. When skin damage reaches deep into the dermis, causing a large area of ​​epidermis defect, the epidermis in that area cannot regenerate and will be replaced by dermal fibroblasts, collagen and proliferating blood vessels, resulting in scars. The degree of acne scars varies greatly among different people and at different ages. There are atrophic scars - pits, hypertrophic scars, etc. Once scars are formed, they are difficult to heal on their own.

Many patients with acne on their faces in winter can't help but squeeze the acne with their hands.
