Cystic acne is a severe (grade IV) acne (commonly known as "pimples"), characterized by nodules and cysts of varying sizes, which are often followed by suppurative infection. After rupture, bloody jelly-like pus often flows out, and sinus tracts and scars are formed later.

1. Endogenous factors

(1) Excessive secretion of androgen is the main cause. Especially during adolescence, excessive secretion of androgen in males and females is the main cause of acne. Androgens can directly stimulate the increase of sebaceous glands, promote the keratinization of hair follicles, clog the pores and cause inflammation, resulting in the formation of acne, pustules, nodules and even cysts.

(2) It is related to the female menstrual cycle. For some women, symptoms worsen before menstruation. It is mainly related to changes in hormone secretion.

(3) Internal disease or functional disorder of the body. Pathologies of organs such as the stomach, kidneys, and liver, especially gastrointestinal dysfunction and constipation, can induce the occurrence of acne.

(4) Mental stress, excessive fatigue, lack of sleep, etc.

(5) Overeating greasy, spicy, sour and other stimulating foods and sweets.

2. Exogenous factors

(1) Long-term contact with grease, asphalt, etc.

(2) Exposure to certain chemicals, such as chlorine, bromine, etc.

(3) Use of certain drugs, such as androgens, sebaceous hormones, lithium, azathioprine, rifampicin, etc.

(4) All factors that can increase skin inflammation, such as alcohol, chili pepper, etc.

1. Systemic treatment

Oral antibiotics, with minocycline being the most commonly used; Chinese medicine tanshinone; and retinoic acid drugs.

2. Local treatment

Topical retinoic acid preparations, topical antimicrobial drugs such as benzoyl peroxide, fusidic acid, mupirocin, selenium disulfide, etc.

3. Physical therapy

Red and blue light irradiation, photodynamic therapy, etc.
